I went to the Nordstrom shoe department last weekend to try on elastic-backed ballet flats and tall black boots without much hope -- I have very narrow heels so I was worried the flats would fall off (I can't wear any regular flats or pumps, since my heels just slip right out), and I have very muscular calves, so I was worried that I wouldn't fit in most of the boots.
But yay Nordstrom -- I came away with both!
For flats, I got the Me Too Lucca in pewter with black tips (I like that they are almost oxford-styling, but in a ballet flat). I've been wearing them all week, and I love them!

For boots, I got the Steve Madden Candance in black. I did have calf-width issues, and none of the zip-up boots worked, and neither did about half of the pull-ons (including none of the Fryes I tried). There were a number of options, though, and I'm just glad that it turns out I'm on the larger side of average, and I don't have to send away for boots. These were just $100 which is great, and which will probably mean I can get another pair of taller boots later this season if I want. They seem to fit fine both over skinnies and bare legs, but I don't have much experience with how boots are supposed to fit, so let me know if you think I'm wrong about either calf fit or if the height will be awkward...

My only concern other than fit is the soles -- they seem rather flimsy. I suppose they'll get through one winter fine, though, which is OK -- I'm still figuring out my style and what I like, so it's probably better for me to buy a cheaper pair this year and play around before I go up to the next price level.
