Loving all the ideas above, and the waistcoat is fab!
Kelly from Alterations Needed used to rock her waistcoats, and I think these outfits look still current (you can change up footwear, and have a look at Angie's blog post about the jeans silhouettes that are on-trend, could be fun to play around with).
I don't have much clothing input because this isn't my style comfort zone, but I think it could work well with the chunky lug soled shoes that are currently trending?
I'm thinking with some chunky soled patent brogues or Oxfords - or even wingtips!
For the record, I am going to have this item tailored. Just below the waist, it sort of flares out a bit to accommodate hips that I don't have. A little tuck on each side should do the trick.
I have a very similar striped vest that I wore recently with a white button down shirt untucked, Avec Les Filles black leather pants, ankle boots w/white soles.... My fashioned forward students loved it!
I wore a similar vest yesterday. I meant to take a photo, but forgot.
I added a trio of vintage brooches to it, and wore it open over a smocked neck denim poet blouse. I paired it with straight leg cotton pants and sandal booties.
