Boy, do I understand this problem, naf. I never knew it was a problem until Angie posted her blog entry about boots not fitting properly at the calf, then I realized that all my boots made me look like I was standing in a hole. My calf is 12 inches, so most boots are several inches too big. As we speak, I am awaiting the return of a pair of boots I left at the cobblers a couple of weeks ago to take in the calf. (He assured me they would be ready by the end of that week, so I am getting a little nervous that he's still not finished, but am hoping they'll be done soon and will look okay.)
Anyway, the only place I know of where you can get boots sized to fit your calf is Duo, which is in England and thus has pricey shipping if you're in the U.S. The boots are not cheap either, but there are sometimes sales. http://www.duoboots.com/
I know Angie has had success with Stuart Weitzman, which reportedly run a little more snug in the calf, but they are a major purchase -- even more so than the Duos.
Best of luck to you. I will post when my boots are back from the cobblers. If you find a good enough sale on boots you love and have a good enough cobbler, it might be worth buying them and having them tailored. It's not cheap, but for me it made more sense than wasting a pair of new boots AND buying another new pair that fit correctly.
