I'm afraid I haven't even tried on the Okalas so can't give a size comparison. I will say that the RW shoes are what I would consider TTS. I normally wear a 6.5 and that's what mine are. What I really like about them is the front does not cut into my bunion AT ALL! I would say they are medium width bordering on slightly wide across the the front of the foot where your big toe/foot joint is and that works really well for me. The ones I'm wearing are actually real suede leather but I note that the blue patent ones are listed as "leather like" so you may want to take that into consideration. It took me a little while to get used to the high back on these and I did have to put a piece of moleskin across the top back so it wouldn't rub but after doing that, super duper comfy.
