I have even stuffed socks or tissue paper in the ankle part of boots because the ankle area loves to bag up. Does anyone else have this issue? I was trying to replace my normal tall (knee) boots last year and searched forever...and never found a pair. As I was trying and trying my shoe guy (the only one I ever work with) at Nordstrom told me to find a slim calf boot, I'd have to go to designer, because they design with the slim woman in mind. (Don't know that I'm all that rich or skinny, but my legs are.)  Angie's suggestions for custom boots were good, but they were just as expensive. I actually stumbled upon a pair of Via Spiga otk boots at the Rack for $150. Seriously, with slim ankles but calves that seem to have grown from cycling (weird, because it seems they've grown more from that than from running!), I'm finding boot fit more challenging than I did in the past. That's part of why I jumped on the Stuart Weitzmans. Those Opening Ceremony ones look great. Yeah, I think this look needs to trickle down into more affordable knockoff territory. I would think it would have wide appeal as it could accommodate those of us with slimmer OR more generous calves and ankles! I guess the Aquatalia came closest but I would prefer not in black and also would like a mid heel vs. high or low.  For now I have my Zara neoprene top booties -- they are not as high as the newer versions but have a bit of that sock boot fit and are great. Mine were pricier than I wanted but in the US you can probably find them cheaper. They're a great height, sleek on the ankle and pass my poison eye for anything western (they look better in person than in the stock photos).
