I have had this kind of burnouts in my career-and so learned hard on my own skin what this means. And I know very well, if there is no help, and you are let alone(like you, and me), you still have to do the job-no matter what....sad but true:-(.&#038;nbsp;I couldn't let go my company of 20 when I was preggy, neither after&#038;nbsp;my DD was born, neither when&#038;nbsp; I was getting daily infusions for my hearing loss, had a car accident...and so on...cuz I have no help.&#038;nbsp;Work simply has to be done....Sorry again, and hope you can deal with all these without damaging your precious health. 				<description>&#060;p&#062;very brief update: I cried all through the meeting: small, soul sucking sobs. I think they finally GOT IT that I needed more resources and needed to know who they were on a daily basis.&#060;br /&#062; In addition I have felt a small bump in my breast that I put off having seen until Monday because my office manager has told me in the past she &#034;prefers' I schedule doctor appointments on the days I don't work.&#060;br /&#062;&#038;nbsp;I decided when I came out of every patient room only to sob in the hallway that I needed a break. I have taken tomorrow off to just relax. will go on date night with hubby Saturday, Sunday to the temple and Monday to have the breast looked at.&#060;br /&#062;Wish me luck and fortitude!

&#060;p&#062;And actually, it's true: People think doctors are professionals, but they're treated like and lead the same lives as hourly-wage workers. My co-workers at the bakery all have two jobs. So that's 16 hours of work a day + commute + not enough money to pay for childcare for the children they don't see. Bakers don't retire; they just drop dead. Pretty brutal.

				<description>&#060;p&#062;That is one of the very first things I learned when I started working at the hospital - the life of a physician is not a glamorous one and the amount of hours worked is insane.&#060;/p&#062;
&#060;p&#062;Sona - first and foremost, you need to look after yourself. You're no good to your family or your patients if you are run ragged. I hope you are able to relay your concerns to Management and that a resolution can be found that works for all.&#060;/p&#062;

It sounds like you handled it with all the grace and skill possible...I am amazed at how much work you did and how well you took care of all your patients. That is an insane amount of work and stress! I agree you should ask for help. Can they hire a locums to fill in? Or reschedule patients or lighten your caseload? If they don't help you out, you need to call in sick. Just because you were able to do it today doesn't mean the workload isn't taking a huge toll on you. You don't want to end up back in the hospital because of your depression so I agree you need to take care of yourself. &#060;/p&#062;
&#060;p&#062;I can totally see where you're coming from, I deal with a lot of the same issues. DD is also in theater camp and I'm planning to go to her performance next Friday even though I'm on call (I'll probably have to go back and stay late at the hospital to finish work). I can't really take time off if I'm sick b/c I'd have to cancel all my patients (many of whom have prepped). No matter how busy I am at work and home I just have to keep doing it because I have no choice. Next month I am on call two weeks and almost all the others in our group will be on vacation that month, and DH is going away for 10 days too so I'll have the three kids at home in the evenings and baby still gets up multiple times a night. I'm so tired, but I can't imagine having all that and having to deal with depression on top of that. So I really sympathize Sona, and wish I could give you a big hug IRL. Please be gentle with yourself and get the help you need. I know you're a strong person and a great caring doctor, but don't let the bean counting managers bully you into compromising your own health.(And I agree, doctors are not rich esp given how hard we have to work).&#060;/p&#062;

				<description>&#060;p&#062;Sona, my freshman roommate (from 36 years ago!) is a family practice doc and she has described similar situations many times. As you well know, you absolutely cannot afford this level of stress. My dear friend eventually found a situation that worked for her--working at an urgent care in an impossibly lovely beach town. She found that an urgent care is a lot easier because you don't have the medically complicated patients to follow. &#038;nbsp;Just the situation at hand. So if management is not receptive I would really urge you to start thinking about what kind of situation would suit your particular talents, needs, and vulnerabilities. You deserve that.

I also think that you have a very good handle on the workload that you can be expected to carry, and you should not hesitate to be firm with the clinic manager on this.  Their mismanagement and poor scheduling have put you in an impossible position.  If you continue to try to do your own work plus that of your missing colleagues you are almost certain to slip up. This is only common sense.   Surely the management does not condone the provision of substandard care to its patients.  Please try to stay strong and fight your corner!&#060;br /&#062;

&#060;p&#062;I think it's crazy that everyone just left you to do all that stuff - you definitely need to take care of #1 (YOU, even though it doesn't feel like you're #1 right now, I'm sure). I think EVERYONE would burn out in this situation.&#038;nbsp;&#060;/p&#062;
&#060;p&#062;I am unsure what the clinic policy is about overbooks, but it sounds like a crazy situation. Maybe the clinic manager will have to put their foot down and limit the # of appointments, and the sicker people will just have to go to the ER.&#038;nbsp;&#060;/p&#062;
&#060;p&#062;Last time I was in family medicine clinic, we went from 9 docs down to 2 (for various reasons) and we shuttled a lot of people to the ER because we couldn't handle it anymore - we had two residents manning the phone to do &#034;triage&#034; - and basically med refills were all called in my our pharmacist (can your pharmacists in the US do this?) as a temporary measure until the docs could follow up. The residents also did all the follow up calls for labs. We made one of the medical students do a lot of scut (consult letters, etc). I dunno if you're at a teaching center, but it might be one of those times to call back the residents - we're usually pretty nice if you buy us dinner, even if you keep us late  <span aria-hidden="true" class="emoticon emoticon-smile icon-emoticon-smile "></span>  - well, at least I am pretty chipper about it!&#038;nbsp;&#060;/p&#062;

&#060;p&#062;And yes, we do make a pittance ... it feels like you give up the best years of your life to be stuck in a hospital or a clinic or studying ... while everyone out there is moving on with their lives. But it's what we have chosen to do, and what we love. If we didn't love it, we wouldn't do it, and we'd run for the hills!&#060;/p&#062;
&#060;p&#062;Take some time off and get help for your clinic. It sounds crazy.&#038;nbsp;&#060;/p&#062;
