A great silhouette for that is the trapeze coat, which sort of billows out from the top. I had one in trench material, and loved it.

Robin, may I suggest the petite sized jackets and vests from L.L. Bean, Eddie Bauer, and Landsend. They might have just the right fit you're looking for!

Definitely an issue for me, and I have never considered alterations. This is such a difficult change and I think it is 50/50 that altering the shoulders might make the rest of the garment look distorted. I look for all in one sleeves, or brands that are generous in the bustline. They're out there - LLBean for one - but there are also plenty of elegant, drapey coats that have room in the bust. Or a very soft shoulder that follows your silhouette. A "sharp" shoulder may no longer be your friend, I'm afraid.

It occurs to me that I have multiple coats with a drawstring waist. This allows me to have room in the bust without a lot of blousiness at the waist.

I also like the drop shoulder suggestion. Good luck shopping!

Tailoring shoulders is expensive and requires an experienced professional. Can be done, though. I often have this fit issue, too. The best cut of outwear for me is raglan or dropped shoulder. Fix shoulders are almost never fit.

I need to refresh my fall and winter outerwear - jackets, vests, or both.

My problem is that I now technically have large breasts on my petite frame.
This is partially due to weight gain, but not totally. Something about my body chemistry changed and, well, they're much bigger than they used to be.

In other words, this is unlikely to change significantly.

This makes shopping for outerwear really dificult.

Most anything I've tried on that fits around my chest is giant in the shoulders.

Are their styles and silhouettes I can be looking for that might fit my shape more easily?

Or is my best option to find something that is close enough to just-right then have it tailored?
Given that it usually has layers, can outerwear even be tailored like that?
