I had custom orthotics and didn't like them. They were too thick even though my podiatrist said they were the thinnest available. Maybe better technology exists now though, who knows.

I'd go get custom orthotics made by a podiatrist. I don’t need orthotics so I can’t speak from firsthand experience, but half my clients wear orthotics. They swear by their custom made versions. However, you might like to try super feet first.

I've used Superfeet inserts (green) for running for many years, and they are great. They are full-length and only can fit in my running shoes. I only started using the 3/4 length inserts for dress shoes last year after finding out about them on this site. I only use them in my Converse and I like them a lot, they make my Converse truly 10 hour shoes. I haven't used them without socks, so I can't speak to blisters and such that Maya mentioned. I have low arches, and one of my feet pronates more than another, and arch support makes a big difference for extensive walking and/or exercise.
I haven't tried Birkenstocks inserts. As for custom orthotics, I was intending to go that route, but never got my act together, and my arches are stronger now, so it hasn't been a problem. Anyway, I would start with Superfeet. You can try them on at the store and see if they work for your arches.

Shana, I have the Superfeet black 3/4 length insoles and I'm kind of torn on them. I chose them because other orthotics tend to be too thick for any non-sneaker shoes, and too squishy for any actual support. The black ones are thin and hard because they are meant for dress shoes. I like the fact that they fit into my shallow flats, and they seem to do their job, but they are extremely hard and kind of rough. I got blisters on my heels the last time I wore them. I wish they had a moleskin cover or something, just to add softness without adding bulk. According to their FAQ, the reason they do not do that is because it would increase the price, but I would have gladly paid a bit more. That might just be me though. They seem to get good reviews from other people. I have tried adding another insole on top but it makes them my shoes too tight.

The different colors represent different uses, like running vs. dress shoes vs. high heels vs. skating, etc. Their website will give you an idea on what would work best. You may end up getting more than one kind for different shoes, or you could try the blue insoles, which are supposedly kind of an "all purpose" insole.

I saw my doctor yesterday about my joint pain and he suggested orthotics for metatarsal support. He told me to try superfeet or the birkenstocks inserts. I looked at the website for the superfeet and I see alot of different options. Any suggestions on which ones are best? Anyone have experience with superfeet or birkenstock inserts? Or should I see a podiatrist?

The funny thing is that his first suggestion was to wear birkenstocks and keens but then I mentioned my new pair of riding boots which is when he brought up the orthotics suggestion.
