I've had that happen a few time in the past year, even with buttons on a very nice wool coat. Lately I've been pre-emptively reinforcing the button stitching myself if I notice they are loose. I would recommend swapping them out if so. I had a coat with metal buttons and they contantly fall off because the metal cuts through the thread a little each time the buttons are used.
