I wear this sort of loose knot all the time in the winter. I think the ends are probably stuffed through the front loop once, but it's hard to tell from the picture. I sometimes do this, it works well with my cotton scarves that already are a bit creased and therefore more voluminous. It's just looped around the neck with the two ends arranged under the loop. It wouldn't work well with a slippery fabric.
