				<description>&#060;p&#062;bj1111 - I just asked him what he thinks is the problem with my back, and is there any other therapy except just treating the pain. And he took it like I don’t trust him and questioning his expertise. I have next appointment with him on Monday but thinking about canceling it.&#060;br /&#062;
Staysfit- unfortunately don’t know anyone who used this place and no one in medical profession. I did asked arround bot no one even heard of this place. I need to check also with my insurance if they would cover for it.

I don't get many chances to speak and practice German, at work we speak only English, at home we speak Croatian, and my social circle is mixture of English and Croatian - I work for UN and hence international community. I think he thought that I am uneducated, took patronizing attitude, did not feel like explaining much to me, and just expected me to follow his therapy without questions. When I did question him, because I felt that his therapy is too aggressive and he started it without waiting for x-rye and MRI results, he got agitated and openly told me to go somewhere else if I don't trust him. So I did. &#060;br /&#062;&#060;/div&#062;&#060;div&#062;&#060;br /&#062;&#060;/div&#062;&#060;div&#062;&#060;br /&#062;&#060;/div&#062;

&#060;b&#062;Laurie &#060;/b&#062;- good luck with your surgery. Happy for you that you finally got the right diagnosis, it can be so frustrating. Neurologist is the next on my list. Few years ago I suffered terrible headaches on my right side and my scalp felt numb. I was barely functioning. But it took months to get correct diagnosis. They put me on strong antibiotics, because they suspected sinus infection. Then they thought that my temple vein (or artery, don't remember now) was inflamed and put me on steroids. I went to eye doctor and got the glasses. But nothing was working. I was frustrated, my doctor was frustrated, I started feeling crazy. Then I went to neurologist and he suspected that it is problem with my jaw. He sent me to physical therapist, the same one I am using now. He immediately diagnosed me with TMJ and showed me some simple exercises. He also suggested that I go to my dentist and get fitted for some silicone thing that I will keep in my mouth while sleeping, because he suspected that I am clenching my teeth in sleep. After only about 2-3 weeks of exercises and sleeping with this thing, pain went away completely and has not returned since. So I know how important but difficult is to get right diagnosis and right treatment.&#038;nbsp; &#060;/p&#062;

				<description>&#060;p&#062;Anchie, I am so sorry that you are so uncomfortable and having such a hard time sorting out how to get the best help for your back pain.  There are so many lovely responses here, and it’s clear from them that there are numerous potential causes for low back pain such as you have described.  (Arthritic changes, disc problems,  muscle imbalances and tightness  IT band, sacroiliac joint syndrome). Do you have any Physical Medicine and Rehab. Specialists?  Maybe it would also be called Sports Medicine.  I believe they are best equipped to sort out this sort of problem because they have excellent functional knowledge of the human body along with pain management training that crosses between exercises/physical rehabilitation and medical interventions including procedures and medications.  If you go to an orthopedic surgeon, they know surgery, and you will most likely end up getting a procedure like the infusions.  If you go to an osteopath, they may miss something like a sacroiliac syndrome on the opposite side causing  paradoxical symptoms, or perhaps functional or actual leg length discrepancy, or piriformis syndrome, etc.  The nice thing about some PMR specialists is that they can refer you to an orthopedic person if needed, or to a good physical therapist, a neurologist or to a more specialized pain management specialist if they feel their training is inadequate. &#060;/p&#062;
&#060;p&#062;It is clear that you do not trust your orthopedic doctor.  I think your instinct is correct but more on that later.  Did you get a clear diagnosis from him/her?  Did he give you the reasons for his treatment strategy?  How about from the other doctor?  If you have a clear diagnosis, and understanding of their recommendations, you can probably do a bit or research and see what the standard of care should be for treatment interventions and also look at the outcomes for those interventions.  Can your doctors give you the Cochrane data or equivalent to support their interventions?  There is a fairly decent evidence base regarding back pain. &#060;/p&#062;
&#060;p&#062;Why didn’t you go to the physical therapist suggested by your orthopedic specialist?  My guess is that you already know the relationship with him is compromised and you have no intention of continuing treatment with him.  Most doctors have a group of professionals they refer to regularly for a specific reason.  The person has a specific skill set that would be helpful, or the person is a good communicator with the doctor, etc.  In your case, your doctor has already hinted that you are failing to follow his treatment plan, and if you go to a different physical therapist this also goes against his recommendation.  Now, if you complain to him that you are not feeling better, he will simply say it is because you failed to follow his instructions.  You can not hold your doctor accountable for his failure to help you.  His belief is that you failed to follow his plan.  He then never feels badly about his treatment approach and can go on thinking that the failure was with you.  He can and will go on treating people the same way——without changing his approach.  His excellent reputation remains intact.  There’s some faulty logic here.....right?!  He’s blaming you for not feeling comfortable with his treatment approach, but did he offer an explanation for it other than it’s what he recommended and you wanted his advice?  Unless he can go back and rescue things by explaining himself, he’s clearly not the sort of doctor you prefer.  He sounds paternalistic and like he expects you to accept what he advises just because he has credentials and experience.  That’s fine for some people, but I don’t think it is for you.  He hasn’t attended to the interpersonal relationship between you, or shown any respect for your remarkable intelligence.

				<description>&#060;p&#062;Hi - I am headed for back surgery in 2 weeks.&#038;nbsp; I have spondylolithesis and stenosis, with disk degeneration that has caused L4 to slip over L5,&#038;nbsp; impinging the nerves down my right leg, with chronic and worsening pain symptoms very similar to what you describe.&#038;nbsp; &#038;nbsp;Of course, something far less dramatic could be causing you to have nerve impingement as RunCarla described,&#038;nbsp; and - happily - resolved.&#038;nbsp; &#038;nbsp;I would strongly advise seeing a neurosurgeon, as was recommended above.&#038;nbsp; Nerve damage can be permanent if untreated.&#060;/p&#062;
&#060;p&#062;Over the last 3.5 years, I have been to 4 physical therapists and a couple of orthopedic guys who were all trying to solve a leg pain problem we thought was due to my hip replacement.&#038;nbsp; &#038;nbsp;Once the back was determined to be the culprit (via MRI and xray) I had 2 spinal injections that did not help.&#038;nbsp; Next stop was the neuro.&#060;/p&#062;
&#060;p&#062;So, so sorry you are suffering this pain.&#038;nbsp; It's utterly maddening to try to figure these things out.&#038;nbsp; Good luck.

I had chronic sciatica-like symptoms, including intermittent numbness, worsening over the past 4 years and had developped a noticible limp with a slight forward pitch when I walked.  After my nurse practitioner ruled out any spine/bone abnormalities, I was cleared to go to a physiotherapist associated with a sports therapy clinic.  2 sessions and a list of exercises was prescribed for a muscle imbalance affecting the right posterior glute chain (pretty much my right leg from calf, right up through to my right deltoid.)  Swollen and tight muscles were squeezing on the sciatic nerve and causing the discomfort.  After 1 month of doing the exercises the pain is gone!  I still have a limp, but recognize the muscle weakness through the right glute and correcting the imbalance will take a while.  The exercises take about 35-45 minutes and I do them every other day before I go to bed.  They are stretches for the quadriceps, illiotibial band, spinal errectors, pelvis and strengthening exercises for the pelvic muscles, glutes, hamstrings, abductors and adductors.  (I have a lot of sports related fitness activity experience, so can be independent for my exercises.  A personal trainer can be a real gift to yourself, if you don't have exercise experience.)  The exercises are not 'fun', but they work!&#060;/p&#062;
&#060;p&#062;Speculation is that years of running on cantilevered highway shoulders in the same direction (facing traffic), may have encouraged the muscle imbalance and jammed up my right hip.  However, sitting for long periods (office desk job, long commute) contributes to further muscle imbalances, tight hip flexors and similar problems that catch up with you as you age.  FWIW, I'm 57 years and worked a desk job for 35+ years.  &#060;/p&#062;
&#060;p&#062;It sounds like a boney abnormality has been ruled out, if your X-rays and MRI are negative.  I don't know the medical hierarchy where you live, but do you have a general doctor or nurse practitioner who manages your care?  Could you have a frank discussion about your concerns?  Possibly formulate a plan for getting better rather than just focusing on making immediate symptoms (pain) go away?

We went to different neurologists and got very different opinions and he was sent to a pain medicine specialist. In the mean time I found the book by Dr. Marcus which I will show you below. He has an office relatively nearby to us so I got DH an appointment. He believes in relaxation exercises and in strengthening the core. All of that is included in his book. I liked that during the appointment he did not promise miracles and also suggested continued follow-up by the neurologist. Dr. Marcus is also a pain management specialist but believes in strengthening the core as tolerated after doing the relaxation exercises. Those really helped my husband improve. I felt very lucky to find Dr. Marcus as he got DH back to his normal condition. My only affiliation with Dr. Marcus is that I am very grateful that he was able to help us and I cannot thank him enough for that. Check out the book if you would like.&#060;br /&#062;
I hope you will find an option that works for you and that you will get better soon.

				<description>&#060;p&#062;My experience with osteopath was he was able to quickly diagnose my severe back and side leg pain as sacroiliitis and give helpful advice. I then pursued further exercises and therapeutic stretches on my own research, which effectively solved the sacroiliac displacement, pain and gait imbalance. This worked along with core strengthening to support the joints and the problem hasn’t returned for 15 years. It did take several months of diligent work to full pain free function.&#060;/p&#062;
&#060;p&#062;An osteopath or chiropractor should be able to assess any misalignments and muscle/tendon strength imbalance issues causing certain types of back pain. That being said, there are also nervous system/spinal issues that may not respond to the same treatments. Good luck choosing your approach and getting better.

He is trying to address this problem first. I also have pain that is traveling down on the right side, in buttock and hip area and numbness in my tights. I really like your approach with personal trainer, and hope that I can find something similar.

&#060;p&#062;Has anyone examined how you walk or looked at your hips when you walk?&#038;nbsp; Back problems can stem from places you would never even consider. I had a severe back&#038;nbsp; problem late last year which was due to weak hips. Standing in front of a mirror you could see one hip higher than the other. I have a fantastic personal trainer who works in conjunction with a chiropractor and physical therapist. My trainer worked out a routine for me that included stretches and hip strengtheners. My chiropractor has also helped with straightening out my hips and I only see him every 6 weeks now as the problem is 99% resolved.&#060;/p&#062;
&#060;p&#062;No back problems, strong glutes, hips, quads,&#038;nbsp; hamstrings, calves, ankles.&#038;nbsp;&#060;/p&#062;
&#060;p&#062;Even problems with your knees or feet can throw your gait off and create lower back problems. I have never regretted taking the holistic approach and convinced my DH to do the same after he received an injection for his sciatica pain which started to travel down his leg. He has not had a single problem ever since he started his strengthening exercises and it has been a few years.

Of course the problem does not disappear forever, but at least when I am all stiff, contracted, in&#038;nbsp;pain (this happens when Winter approches - humidity and cold do not help) I go to him and come out relaxed. I like it that he considers the state of health of the whole organism. He&#038;nbsp;gives me suggestions about posture, sleeping habits, exercise, eating (that I not always&#038;nbsp;follow!). It could be possible that your osteopath, more than Xrays and exams, &#038;nbsp;will be able to understand where the problem is! I am often stiff in my back&#038;nbsp;and a few years ago I started to go to an&#038;nbsp;osteopath that an enthusiastic friend suggested to me (he solved her terrible shoulder pain in a few treatments).&#038;nbsp;It is very relaxing&#038;nbsp; and I&#038;nbsp;like&#038;nbsp;the &#038;nbsp;approach.&#038;nbsp;Yoga also helps.

				<description>&#060;p&#062;I mentioned my lower back pain on the main forum and some of you mentioned to have experience with the same. I would like to hear your opinion about my situation. I have been going to orthopedic dr that was highly recommended to me. He is orthopedic surgeon and pain management specialist. I had many rounds of infiltrations in my back, infusion for the pain, and he also gave me back belt and prescribed morphium as pain killer which I did not take. He became very upset with me when he asked about morphium and I said I am not taking it. He said why I am coming to him if I don’t want to listen to him and follow his therapy. A few days ago he finally decided that I can start with physical therapy. His ordination also provides this, but I decided to go with physical therapist that I used before. And here is my problem. When I told my story to dr there, who is physical therapy specialist but also osteopath and manipulation&#060;br /&#062;
specialist, he asked me to stop my therapy and not take more infiltrations or infusions (I had few more rounds to go) and not to use back belt. He said that this is just poisoning my body, and not treating the cause. He performed manipulation on my spine ( he checked my x-ray and MRI results before that), and I had 2 osteopathy sessions already. He even told me not to use back pain creams from the pharmacy but told me to make my own, or to use traditional chinese one. I am really confused now. Which doctor should I listen to? It seems that they are complete opposites. I like philosophy of the second one - everything natural, treating the cause etc. Should I listen to him and stop my therapy with the first doctor? Does osteopathy even work? Does anyone have experience with it?
