Yep, I see Lyn has nailed my BFFs schedule during her residency. Even though she is now a general surgeon with a practice, call schedule is still a factor, and often a gigantic PITA.

Anna, it's a shame your colleague can't be a bit more compassionate. I can never understand that kind of thing. I would absolutely hate to think that I'm causing someone else to work longer hours, and I'd be as appreciative as I could!

Wow. Especially after reading lyn*'s description, I feel like call cannot possibly be the best solution anyway. I mean, when I don't get enough sleep, I get completely woolly-headed and start firing off emails to the wrong places. I don't think I like the idea of putting someone in charge of medical care in a situation where they have to go 24 hours without some decent REM. Yeesh.

I figured that the situation was like that Anna, but I appreciate you explaining. At least when the SO gets called in they pay him for most of his commute because of where he lives, and mileage if he drives his own vehicle. Usually they don't have a official call since they run 24-7 coverage, but I know other groups in the area do "call time" and being on call is usually half your rate of pay in his field, but much less with my dad. Still, she could be just socially reclusive and not a total nincompoop, either way, I feel sorry for you dealing with her atm.

I get a whole $3 an hour for carrying the pager. I do not get travel time or mileage. I only get OT or comp time for the actual time in the hospital.
I would rather be out with my friends having drinks, hiking or boating, especially this summer.

I shouldn't really answer for Anna's call experience, but our call works something like this - we work a normal day from 8 to 5, and then we get hand over at 4:30 or so. Our call shift starts at 5. Technically, if there's nothing going on we can go for dinner. This never happens. You start with your pager going off and we are responsible for 3-5 wards in addition to whatever happens in the emergency department and whatever new consultations happen to come in from the rest of the hospital.

Most shifts, I start at 4:45 (If my clinic is over; if not, I still have to go back and see my clinic patients which are booked according to the staff person I'm working with; so it's not like I can stop booking at 3:30 pm), answering the phone to deal with day to day stuff that the day staff didn't get to, and then at 5:00 I get anywhere from 1 to 7 consults of patients I need to see that night. As I'm working through the 7 consults (taking from 1 hour to 3 hrs each), I get additional consultations coming in from the ER or the rest of the hospital. Time that with pages that come in as often as 27 in 7 minutes (oh, neurology, how I hated you), ranging from things like "Can this patient have Tylenol" to "YOU MUST SEE THIS PATIENT RIGHT NOW!!!!" to "This patient isn't breathing!" - you have to juggle family meetings, nursing concerns, patient concerns and seeing new patients, as well as holding off as many consults from the ER as possible because you are already swamped with everything else that is coming in.

By the time a shift ends, you have done about 26 to 28 hours of patient care. Before you can go home, you have to go and do all the paperwork and dictations from the night before, so it can be hour 32 or 33 before we actually "go home". And we are expected to work the next day.

And we are paid about $127 or so for this.

And one person handles all of this.

Perhaps I am misunderstanding-- not in the medical profession. Where I come from, when you are on call you get paid to be on-call (i.e. sitting at work or at home, but not working) then paid more if you do work. Is this different for those in the medical field?

If so, she could just be a homebody type/not one with many plans. I know certain family members that work call for emergency services that don't mind being on call because they can still sit at home (which was their whole plan) and then might get time-and-a-half or double overtime if they have to work. They are also those that usually don't drink or drink often, so not being able to because of call doesn't affect their enjoyment. For example, my father plows snow, and being on call means he gets to hang out and watch TV, but if there is a storm usually gets a full 8+ hours of pay by the time he commutes, gets the equipment out, plows multiple times, and then maintains the equipment. He doesn't usually drink, and doesn't mind being on call a couple weekends a month because he is antisocial.

For you though, I totally agree that it sucks. I am not a fan of when the SO is on call, because it means we can't go anywhere or have solid plans, and often have the entire day disrupted for only a short period of time/pay--- aka the 2 am call in that only takes 3 hours.

Ohmigosh. My spiritual director in college, a nun, once told me to think of everyone as sheep. Lot of bleating and noise, you just herd them here and then there and don't try to make sense of the baa-ing. Because. Call. Sucks. You have to put your life on hold and you may or may not get the hours (and attendant pay). And even if you do get the hours, you have to put your life on hold. It. Sucks.

My English ex-boss was very annoying in the same way. If a truck ran over your leg, he'd say, "Well, it could have been both legs!"

That sounds to me like the comments from a woman who is feeling guilty about the extra burden on you and is looking for a way to rationalize that there is atleast some payoff to you. I'm sure it would feel much better to you if she just acknowledged the load you're under and showed some appreciation, but I've noticed when people are feeling guilty they aren't usually the best communicators.

Everyone I've ever known who took call (family docs and pediatricians) has hated, hated, hated it. At first I didn't understand (because sometimes the duty can be light) but after a while the unanimity convinced me. (Sorry for being slow.) Also, sorry you're on call tonight and over the long weekend!!!
