I have the same problem, I am between a 7 and 7.5 but the 7 was too tight in the toes and the 7.5 I have a tiny bit of heel gap. But it doesn't fall off my foot when I walk. I opted for comfort and went with the larger size. I haven't needed to put a heel grip (most of my pumps do have a gap in the back for some reason). Ordered both 5.5 and 6. the 5.5 is ok for length but a bit tight on the front, i feel my feet is squeezed a bit, may need to wear a few times to stretch it; the 6 is ok for front, very comfortable, but a bit big for length(half finger), when i wear it, it doesn't fall off. I think i'm in between 5.5 and 6. I wonder does patent leather shoe stretch? Which size shall i keep? </p>
