They measure you with a bra on, then bring you a bunch to try on. So the fitter saw me with my top off and bra on, but left the room when I changed bras. I had a fitting in Nordstrom where they assessed my size with my bra on and then bought me a few sizes and reassessed it and got me new sizes. After that it was a matter of narrowing down the style. I also had a fitting in Intimacy in New York where I had to remove even the bra for the fitter to settle upon a size. Nordstrom is a good place to get one (I've gotten it done there before). I've heard mixed reviews about Victoria's Secret (they try to fit people into their limited range of sizes).
